All-on-4 implant treatment with fixed prosthesis application in Maltepe Istanbul

Did You Know?

All-on-4 and All-on-6 are full-arch implant techniques in which 4 or 6 implants carry a complete fixed prosthesis on a single jaw. Two angled posterior implants typically remove the need for bone grafting, allowing same-day temporary teeth ...

All-on-4 / All-on-6 Implants Istanbul — Permanent Solution for Full Edentulism

If you have lost all your teeth or your remaining teeth are no longer functional, you may want to eliminate the hassle of removable dentures. All-on-4 and All-on-6 implant treatments are among today's most successful methods for those seeking a fixed, permanent solution for full-arch edentulism. At our Maltepe clinic, the procedure is led by Dr. Aykut Gürel — Oral & Maxillofacial Surgeon, and is part of our medical tourism programme for international patients.

03

What Is the All-on-4 Implant System?

All-on-4 is a revolutionary method that enables a fixed prosthesis to be attached to four implants placed in the jawbone. Developed by Nobel Biocare, the greatest advantage of this technique is that the angled placement of the posterior implants typically eliminates the need for bone grafting.

Key Features of All-on-4:

  • Only 4 implants are placed per jaw
  • Posterior implants are placed at a 30-45 degree angle
  • A temporary prosthesis can often be fitted on the same day
  • The need for bone grafting is minimal
04

What Is the All-on-6 Implant System?

All-on-6 is an advanced method that provides greater support by using six implants. It is particularly preferred for the upper jaw or in situations requiring higher masticatory force. This method distributes chewing forces over a wider area, ensuring long-lasting prosthesis durability.

Advantages of All-on-6:

  • Maximum stability with 6 implants
  • Better distribution of chewing forces
  • More reliable results in the upper jaw
  • Greater long-term durability

Frequently Asked Questions

Common Questions

What is the main difference between All-on-4 and All-on-6?
All-on-4 uses 4 implants, while All-on-6 uses 6 implants. All-on-6 provides more support and is especially preferred for the upper jaw. It distributes chewing forces over a wider area, delivering more stable long-term results.
Can teeth be fitted on the same day?
Yes, if bone quality and implant stability are adequate, a temporary prosthesis can be fitted on the same day. This is called "immediate loading." A soft diet program is followed for the first 3-6 months.
I smoke. Can I still receive treatment?
Smoking negatively affects implant success but does not completely prevent treatment. It is recommended that you quit or at least reduce your daily consumption before treatment. For detailed information, read our article on smoking and implants.
I have insufficient bone. Can it still be done?
One of the greatest advantages of All-on-4 and All-on-6 techniques is that they can be performed even in patients with bone deficiency. Thanks to angled implant placement and utilization of anatomical structures, bone grafting is not required in most cases.
How long does the prosthesis last?
The implants are permanent for life. The prosthetic component can last 15-25 years with proper care. After this period, only the superstructure may need replacement while the implants remain in place. Regular check-ups and good oral hygiene extend longevity.
How much does All-on-4 cost in Istanbul vs the UK?
A single-arch All-on-4 in Istanbul costs £4,100–7,300 / €4,800–8,500 (TRY 80,000–120,000), compared to £15,000–20,000 in the UK or €18,000–25,000 in Germany. That is a saving of roughly 60–70% for the same materials and surgical technique. Both arches: TRY 160,000–240,000 (£8,200–14,600 / €9,600–17,000).
How long do I need to stay in Istanbul for All-on-4?
The surgical visit is 5–7 days: arrival, CBCT, surgery on day 2, three days of post-op review and bite adjustment of the temporary prosthesis, and travel home on day 7. The final prosthesis is fitted at a second visit of 3–5 days, 4–6 months later. Some patients have the final prosthesis made by their local dentist using the digital files we provide, but the second Istanbul visit is recommended for the best fit.
Will I leave Istanbul with teeth?
Yes — in suitable cases (most cases) we fit a same-day temporary fixed prosthesis during the surgical week. You leave Istanbul with a complete, fixed set of teeth and travel home with no gaps. The final prosthesis is made at the second visit after osseointegration is complete.
Do you accept international patients for All-on-4?
Yes. We provide written estimates in EUR or GBP, support with travel and accommodation coordination, airport transfer, and 12 months of WhatsApp / email follow-up after you return home. Translation is provided throughout your visit. Our four dentists work in English at B2 level. See our Dental Tourism Istanbul guide for the full process.
What if I have a problem after I return home?
We provide 12 months of WhatsApp and email follow-up at no extra cost. You receive detailed clinical notes and X-ray copies in English so any local dentist can step in for routine concerns. For warranty cases (e.g., implant failure within the manufacturer warranty), we cover the cost of the replacement implant; you arrange a return visit for placement.
Which implant brand do you use for All-on-4?
We tailor the brand to your bone quality, immediate-loading needs and budget. Straumann SLActive is preferred for atrophic cases or where same-day temporary teeth are critical (highest primary stability). Megagen Anyridge offers similar primary stability at a lower price point. Osstem TS IV SA is the value option for standard cases. All brands carry a manufacturer lifetime warranty on the implant body.
Is bone grafting needed for All-on-4?
In most cases, no — that is the central advantage of All-on-4. The two posterior implants are placed at a 30-45° angle, which lets them anchor in available bone without sinus lift or graft. Where atrophy is severe, we may add a sinus lift, a small graft or a zygomatic implant for one or two positions. The CBCT scan tells us in advance.
Can I combine the surgical visit with a holiday?
Cautiously, yes — after the surgical week. During the 5–7 day surgical block, light walking and light sightseeing are fine; heavy meals, hot tubs, alcohol and strenuous activity should be avoided for 48–72 hours after surgery. Many patients build a 10-12 day trip with the second half spent exploring Istanbul.
What is the success rate of All-on-4?
Long-term studies show 5-year survival of 94–98% and 10-year survival of 91–95% for the angled-implant technique (Maló et al., systematic reviews 2019). Success depends on bone quality, surgical technique, immediate-loading discipline, and aftercare. Smoking, uncontrolled diabetes and poor hygiene reduce success rates. ---
